You may qualify for SSBCI if you have a high risk for hospitalization and require intensive care coordination to manage chronic conditions such as Chronic Kidney Diseases, Chronic Lung Disorders, Cardiovascular Disorders, Chronic Heart Failure, or Diabetes. Every year, Medicare evaluates plans based on a 5-star rating system. Star rating is for contract year 2026.A beneficiary may use the 5-star SEP only one time from December 8 through November 30 of the following year in which the organization has been granted a 5-star overall rating. The enrollment effective date is the first of the month following the month in which the plan receives the enrollment request.
